Learning Disabilities

Our care staff specialises in positive behavioural support, and is qualified to deliver it to those with learning disabilities. Our care staff is always monitoring the emotional, psychological, and behavioural needs of the individuals we support. A holistic approach is applied, which takes into account an individual’s life history, personality, physical health, and emotional needs.

Passion Home Care has implemented a person-centred strategy that encourages participation from autistic individuals, family members, and other healthcare professionals in order to provide clients with support in the development of functional abilities and skills that improve the overall quality of their lives.

Physical Disabilities

Whether temporary or permanent, inherited or acquired, we support people with all types of physical disabilities. We also work with individuals suffering from cerebral palsy, spinal cord injuries, amputation, multiple sclerosis, spina bifida, musculoskeletal injuries, and muscular dystrophy.

We provide services from a few hours a week to 24 hours a day, seven days a week. Our plans are always individualised for each individual and those closest to them, ensuring that we understand the problems they’ve previously conquered, as well as their urgent needs and wants, as well as their long-term ambitions.

Mental Health Needs

We urge individuals to participate in meaningful conversation about important topics that affect how we live our lives. In addition to assisting with the development of long-term self-management techniques, our expert support may help with the assessment of personal needs, difficulties, and objectives.

We work with social services and community home teams to create customised care plans that are then monitored on an ongoing basis. We utilise several techniques, such as Recovery Star, to assess our progress.

When individuals determine for themselves what they want to achieve, it helps to sustain motivation, enhance skills, build self-confidence, and assist in determining what their objectives are and how they may be attained.

How we support:

Assistance with Daily Living Activities:

We assist individuals with daily living activities such as bathing, clothing, grooming, and food preparation. These activities promote independence, safety, and health.

Medication Management:

Passion Home Care helps individuals manage their prescriptions, ensuring they take them on time and in the recommended doses. Preventing pharmaceutical misuse’s consequences.

Emotional Support

We provide emotional support. Our carers understand specific issues individuals encounter and can listen, soothe, and encourage when required.

Social Engagement:

Passion Home Care promotes social participation and inclusion among individuals in our care. We organise social activities, events, and trips to improve relationships, friendships, and the quality of life.

Health Monitoring and Advocacy:

Individuals under our care are routinely monitored. Tracking vital indicators, aiding with medical visits, and collaborating with healthcare experts. We advocate for our clients, ensuring the best medical care.

Skill-building and Education:

Passion Home Care provides skill-building and educational programmes to support individuals.  These include life skills, vocational training, and educational programmes.

Transport Assistance:

We assist individuals with transportation needs, ensuring they can go to medical appointments, community events, and other important places. This fosters independence and mobility.

Family involvement and support

Our organisation values family engagement in the care and support of individuals. To keep family members informed and active in care, they support, resource, and communicate with them.
